Life Events

Over the course of your SDRS membership, you may experience life events that require you to report name changes and/or beneficiary changes to the SDRS office. Marriages, divorces, births, adoptions, and deaths all create changes in family status and beneficiary designations. To update your SDRS record, you must complete various forms and provide proper documentation.


Marriages: To update marital status and name (if applicable) due to marriage, complete SDRS Form E-2 Notice of Enrollment Change and provide SDRS with a photocopy of the marriage certificate. You will also want to complete SDRS Form E-5 Beneficiary Designation to ensure your beneficiaries are up-to-date.

Divorces
: To update marital status and name (if applicable) due to divorce, complete SDRS Form E-2 Notice of Enrollment Change and provide SDRS with a photocopy of the divorce decree. You will also want to complete SDRS Form E-5 Beneficiary Designation to ensure your beneficiaries are up-to-date.

Births/Adoptions/Deaths

Designating Minor Children as Beneficiaries: SDRS cannot make payments directly to minors. If any of your beneficiaries are minor children, you may also wish to complete SDRS Form E-5A Transfer to Minor to name a custodian to manage SDRS assets belonging to the minor until the minor reaches age 18.
